Clear copy of
your photo passport. Passport must be valid for at least 6 months from the day of departure. For Cuban born passengers please call. |

Charter
operator reservation form Complete information and signed by the passenger. In case of minors that can not sign the reservation must be signed by the parent or guardian. |

Affidavit Complete form and sign. |

Payment Please make full payment for the service at least 2 weeks before departure. Payment can be made with personal check, cashier check or money order. No credit card payment is accepted. |

* We need originals of all signed documents
no later than 10 days before departure. *Children must have authorization from both parents to travel outside the US. Some airlines require a notarized letter signed by both parents approving such travel. We do not process reservations if any of the information is missing from the forms mentioned above. Missing information will delay the confirmation of your request. |
